There are three possible paths for requesting a passport name change. The path you choose depends on how long it has been since you obtained your passport and whether you have legal documentation or not of the name change. If your name was changed within one year of your passport´s issuance and you can document that change, then there is no charge. Otherwise, you will have to pay a fee. Be sure to verify which situation applies to you, what form you need to complete and what documentation to present.
